IVF and ICSI Treatment

In-Vitro Fertilisation (IVF)

In-vitro fertilization means the fertilization of the egg happens in the controlled laboratory condition. In this process the eggs are collected from the women’s body and are mixed with the partner sperm in the petri dish and allow them to fertilize spontaneously.

Potential candidate for IVF

  • Tubal factor (absent, damaged, blocked tubes)
  • Endometriosis
  • Unexplained infertility
  • Male factor infertility
  • Advanced maternal age or low ovarian reserve
  • Genetic abnormalities

Intra-Cytoplasmic Sperm Injection (ICSI)

One of the major breakthroughs in assisted reproductive technology is the invention of ICSI technique. ICSI is the procedure where a single sperm is injected directly into a single egg. The resulting embryo is then placed into the woman’s womb.

Potential candidates for ICSI

  • Male factor infertility (No sperm/ Low sperm count or low sperm motility)
  • Abnormal sperm morphology
  • Men who have undergone vasectomy
  • In surgically retrieved sperm (PESA, TESA, TESE, MESA)
  • Prior IVF failure
  • Advanced maternal age or low ovarian reserve
  • Very few eggs retrieved (less than 3 eggs)
  • Avoidance of transmission of infectious disease (eg.HIV)
IVF-ICSI

Basic Stages of IVF/ICSI involves:            

  1. Suppression of natural cycle
  2. Growth and development of multiple follicles
  3. Monitoring of follicular development
  4. Trigger injections
  5. Trans-vaginal Egg retrieval
  6. Sperm processing on the day of egg collection
  7. Either IVF/ICSI procedure
  8. Assessment of Embryo development
  9. Transfer of embryo (D2/D3/D5)
  10. Progesterone support
  11. Blood Pregnancy Test

IVF and ICSI Treatment is the advanced fertility treatment. Even in the current era of advanced ART techniques, we face a challenge of “ Implantation Failure “ limiting success rate of IVF to 50-60 %. Thus having IVF can be physically and emotionally draining for the couple. Step 1 : Preparation Phase

–           Detail discussion with Fertility Specialist about need of treatment , alternative options ( shared decision ), individualised success rate of the treatment , any adverse effects and the process of the treatment

–           Necessary tests for couple

–           Lifestyle modifications like healthy diet, regular physical exercise, mind relaxation , control of existing medical conditions, vaccinations if any, stopping alcohol intake and smoking if any

–           Counselling  sessions by Fertility Specialist, Embryologist and IVF Nurse

–           Understanding and signing consents for the procedures

Step 2 : Actual Treatment phase

–           Medications to stimulate the ovaries to produce multiple eggs

–           Monitoring by blood tests and sonography

–           Collecting eggs trans vaginally under ultrasound guidance with minimal anaesthesia

–           Collecting sperms on same day

–           Embryos are formed in laboratory by advanced techniques (ICSI)

–           Embryos are transferred to womb (Uterus) under ultrasound guidance

–           Medications to support implantation process

–           Pregnancy test after 15 days

Step 3 : Counselling after IVF failure

–           It’s very important phase of treatment where at most support is required to tide over the crisis of unsuccessful IVF cycle.

–           We discuss in details about the possible reasons and way forward.

Throughout the process the Expert team of fertility specialist, Embryologist and IVF nurse provides all information to couple, addresses their concerns and supports them emotionally.
